NASAL RECONSTRUCTION USING SKIN FLAP


Abstract
Nasal defect or deformity may impose significant adverse effects on one¢¥s appearance because of its location on the midface. So it is needed that the best method of reconstruction is performed according to the degree of defect or deformity.
During the past five years, We experienced ten patients with nasal defects who needed skin flaps for their reconstruction.
Four forehead flaps, three nasolabial flaps, a Washio flap, and a scalping flap were used fo1 outer lining in these patients, We also used seven local turn-down flaps, a nasolabial turn down flap and a forehead flap for inner lining. In two cases, a L-strait iliac bone graft and a septal hinge flap were put for midline support, but any lateral support was not necessary for prevention of airway collapse.
From our experiences, forehead flap, nasolabial flap, Washio flap seem to be suitable for outer lining in nasal ala or lobule reconstruction, and in most cases local turn-down flaps can be used for inner lining. In subtotal or total nasal reconstruction, scalping flap, expanded fore head flap and free flap can be provided as donor tissues for outer lining and we can select one of skin graft, local turn-down flap or other skin flap for inner lining. In addition midline skeletal support is usually needed for nasal projection.
